When applying for food or utility assistance, our organization collects the following information: full name, address, birthdate, a copy of a utility bill (if utility assistance is needed, pay stubs (utility assistance only, to verify you qualify for assistance), and phone number. This information is securely collected on Jotform and stored on Salesforce. No information is shared outside the organization. 

SMS consent is not shared with third parties. No information provided is shared with outside individuals or organizations.
